Jay Lee Forman

Our loving son and brother passed away May 30, 2018, of natural causes.

Jay was born December 12, 1959 to Calvin and Maurine Forman, and raised in Herriman, UT. He is survived by his mother, Maurine, his brothers, Bill (Bea), Dennis (Jackie), Bob (Nancy) and sister, Lyn. He is also survived by numerous aunts, uncles, nieces and nephews. Preceded in death by his father and sister-in-law.

Jay worked construction most of his life. After he retired, he became a great handyman to many. There was never a project too big or too small for Jay. He had the instinct to know how to master the project, and use his large network of friends, relatives and co-workers to get the job done.

Jay's friends were important to him. He enjoyed the many barbecues, fishing trips, golf trips to Mesquite and Wendover, and Fantasy Football.

A memorial service will be held on Monday, June 4th at 11:00 a.m. at Jordan Oaks 3rd Ward, 8117 South Leslie Drive, West Jordan, UT 84088. There will be no visitation. Interment - Herriman City Cemetery.
