Derik Lee Provstgaard

Our a beloved son and brother Derik Lee Provstgaard has passed away at the age of 31. His life was way to short and ended tragically. He was loved and will be missed! He is survived by his parents Dana Runnells, Scott Provstgaard, and his brothers and sisters, Jason Provstgaard, Matthew Barlow, Michael Byrd, Tyler Jorgensen, Chanelle Arriaga, Brandi Byrd. His services will be held at the Rose park 6th Ward on May 3rd�with a viewing at 10:00 a.m. and funeral services at 11:00 a.m., followed by a luncheon at the church. The address is1155 N 1200 W. Enter on the South end of the building. Afterwards we will continue to the graveside services at the Valley View Memorial 4335 W 4100 S West Valley, Utah.
